Acne is more than just a surface-level skin issue; it’s a complex condition that can greatly impact one’s self-esteem and quality of life. Appearing as pimples, blackheads, and painful cysts or nodules on areas like the face, back, shoulders, and chest, acne is caused by clogged hair follicles filled with oil and dead skin cells. It varies from mild to severe and can extend into adulthood. Beyond physical discomfort and scarring, acne often carries a psychological toll, leading to social withdrawal, anxiety, and depression. Its visibility, particularly on the face, can make social interactions challenging, affecting both personal and professional relationships. The development of acne is influenced by various factors such as hormonal changes, diet, stress and genetics. Hormonal shifts during puberty, menstrual cycles, pregnancy, or medication use can increase oil production, worsening acne. Diets high in dairy and sugar may also aggravate it. While stress doesn’t directly cause acne, it can make it worse by raising cortisol levels, which can boost oil production. Although there’s no definitive cure for acne, understanding these factors and managing them with lifestyle changes, medical treatments, and skincare routines can help control breakouts. Traditional Chinese Medicine (TCM) approaches acne from a holistic perspective, emphasising dietary adjustments, herbal treatments, and acupuncture to restore internal balance and support skin health, offering a complementary or alternative option to standard treatments.

Types of Acne
Whiteheads and blackheads, collectively known as comedones, are often the first visible signs of acne. Whiteheads form when pores become clogged with sebum (skin oil) and dead skin cells, creating small, closed, flesh-colored bumps. Blackheads, meanwhile, are open at the skin’s surface where exposure to air causes the clog to appear black. Both types can appear on the face, chest, back, and shoulders—areas with highly active sebaceous glands. Without proper treatment, these early signs of acne can progress, affecting skin texture and appearance.
As acne worsens, it may develop into papules and pustules. Papules are small, red, raised bumps resulting from inflamed or infected hair follicles. They can be tender to the touch and indicate a moderate level of acne. Pustules are similar but contain a white or yellow pus-filled center surrounded by a red ring due to inflammation. Both types contribute to acne discomfort and, if left untreated, can lead to scarring and emotional distress. Proper care and treatment are essential to control these symptoms and prevent further skin damage.
Cysts and nodules represent severe forms of acne that reach deep within the skin. Cysts are large, painful, pus-filled lesions that can leave lasting scars. Nodules are firm, painful lumps under the skin caused by severe inflammation within sebaceous glands and hair follicles. Both types require professional medical treatment, as they not only cause significant discomfort but also emotional stress. Addressing cysts and nodules early can help prevent scarring and support skin recovery.
Post-inflammatory hyperpigmentation (PIH) often follows acne, leaving dark spots or patches where lesions have healed. PIH results from excess melanin produced in response to inflammation, causing uneven skin tone that can persist for months or even years. Although not a symptom of active acne, PIH can be as distressing as acne itself. Managing hyperpigmentation involves sun protection, topical treatments to lighten dark spots, and patience as the skin gradually returns to an even tone.
